In addition to the medical documentation, it is necessary to prepare the legal documents that must be submitted before traveling abroad to initiate a surrogacy process.
As per the law governing surrogacy in Ukraine, it is a requirement that the intended parents be a legally married couple. Therefore, the first document needed is the legalized and apostilled marriage certificate. In the case of Georgia, legal marriage is not necessary, but proof of cohabitation is required, showing a shared address.
Secondly, valid passports for both the intended mother and father must be presented. Both sets of documents should be scanned and emailed to the surrogacy center for approval.
If the family has opted for egg donation and, in this case, only the intended father plans to make the initial trip, additional documents need to be included in the pre-travel submission: a copy of the mother's passport legalized and apostilled and a power of attorney in favor of the surrogacy center signed by the mother.
These same original documents will need to be brought on the first trip to Ukraine and Georgia to be presented to the notary of the surrogacy center when signing contracts and creating the family folder.
